Office Policies

Before scheduling below, review the required office forms/policies. By scheduling an appointment below, you are attesting to having read, reviewed, and agreeing to our office policies. If you have any concerns about our office policies, you should NOT schedule an appointment. We only evaluate patients 18 years of age and older. Appointments should only be scheduled by the patient or legal guardian if applicable. Patients may bring one ADULT guest.

Cosmetic / Self-Pay Pricing

Cosmetic evaluations are not covered by medical insurance. The new patient fee is $255 and return visits are $195. There is a surcharge for procedures. Additional information regarding self-pay pricing can be found here.

Medical Insurance

All claims in our office are processed through medical insurance, not vision insurance.

If you are scheduling an insurance based visit, the amount you will owe for the visit will depend on your insurance plan and may include a co-pay, unmet deductible, or co-insurance. If you have questions about how your insurance plan works and what your financial responsibility may be in our office, please contact your insurance company by calling the number on the back of your card. We will also provide you with an estimate for the amount owed once you provide your insurance information. You can upload your insurance card when scheduling via the web below or you can send us photos of the front and back of your insurance card by text message or by using our secure app for HIPAA compliant communication.

If you have any questions about whether your insurance plan can be accepted in our office as being in-network, please contact your insurance company by calling the number on the back of your card.

Visits for cosmetic concerns are not covered by medical insurance and are considered self-pay.

Reason For Visit

Our practice is focused on a core set of procedures (within the field of oculoplastic surgery) that we perform regularly. Before scheduling, please review the list of procedures/conditions to ensure your condition is one that we evaluate/treat in our office.

When entering the 'Visit Reason' field either by web-based or text based scheduling, please be as specific as possible.

Under ‘Visit Reason’, enter your insurance ID if applicable and also upload or send us photos of your insurance cards (if applicable) in the appropriate field. Depending on your insurance plan, it may be called one of the following on the front of your card: member ID, policy ID, subscriber ID.

Health and Safety

Before scheduling below, please click to review the COVID-19 screening and COVID-19 consent for treatment. You will ultimately sign these forms during the check-in process. Masks are required. Our waiting room is closed. You will text us when you park and will wait in the car until we are ready for you.

Confirmation

As your appointment nears, you will get confirmation texts from AI Virtual Assistant to confirm your appointment. The text will come from 984-279-0313. If you do not confirm at least 24 hours prior to the appointment, your appointment may be canceled.

Late Cancelations/No Shows

Cancelations with less than 24 hours notice are subject to a fee of $50. No shows are subject to a fee of $100.
